In 2023, Paul Lonergan embarked on a quest to uncover the source of his persistent throat discomfort. Following a series of medical consultations, the Toronto resident received a startling revelation: a sizable tumor was detected at the base of his tongue. “I realized I had a battle ahead of me,” expressed the 68-year-old.

Enduring a taxing regimen of seven weeks of radiation and chemotherapy, Lonergan underwent treatments that led to multiple stays in the intensive care unit to eradicate all traces of his tumor. However, the question remained: Was his cancer truly eradicated?

Participating in a clinical trial offered by his healthcare team, Lonergan underwent a “liquid biopsy” to determine the answer. This innovative method of cancer detection utilizes simple blood tests to identify traces of tumor DNA undetectable through conventional scans and examinations. Results revealed that despite successful treatment of his tongue cancer, Lonergan still harbored tumor remnants in his bloodstream, indicating a potential resurgence of the disease.

Subsequently, Lonergan was provided with experimental immunotherapy, leading to his current cancer-free status as confirmed by subsequent liquid biopsy assessments scouring his blood for any minute signs of the illness.

Although limited in accessibility to few Canadians, this evolving approach to cancer monitoring primarily exists within the realm of clinical trials or upscale private clinics. Nevertheless, there is optimism for broader implementation in the future.

Dr. Ramy Saleh, a cancer researcher and medical oncologist at McGill University Health Centre in Montreal, emphasized the transformative potential of this approach in revolutionizing disease recurrence surveillance by healthcare providers.

A Toronto-based team has initiated an extensive multi-year research initiative to explore the integration of this emerging technology into routine patient care. The overarching aim is to enhance the protection of cancer survivors against disease recurrence while potentially sparing them from costly and invasive treatments.

Under the leadership of scientists from University Health Network (UHN) in Toronto, a clinical trial, named SHERLOCK, will administer liquid biopsies to over 7,000 cancer survivors who have undergone traditional curative treatments for various cancer types over the next five years. The study, spearheaded by Dr. Lillian Siu, aims to detect circulating tumor DNA indicative of potential cancer relapse.

Designed to identify hidden disease signals, the SHERLOCK project strives to detect cancer in the blood before it becomes visible through routine clinical examinations or scans, allowing for prompt intervention and improved treatment outcomes for patients.

The evolving technology of liquid biopsies, a diagnostic tool introduced just over a decade ago, has shown promise in early cancer detection and disease monitoring post-surgery. Ongoing research indicates a correlation between early detection through liquid biopsies and enhanced survival rates for various cancer types.

Despite the potential benefits, challenges and ethical considerations encompass the use of liquid biopsies for cancer survivors. The technology’s imperfections, such as false positives and negatives, highlight the emotional burden and complex decision-making patients may face when relying on these tests for disease monitoring.

Continuous research and refinement are imperative as the field of liquid biopsies advances, with the ultimate goal of making these tests a routine practice in cancer care, offering substantial benefits to patients while optimizing healthcare resources.
